Moreover, in her November 30, 2006 letter, LeAnn advised Conseco, for the first time, that, although her last day of work was February 4, 2003, her automatic payroll deductions had continued until June 14, 2003, because she used her accrued sick and annual leave from February 4, 2003, until June 14, 2003, when her application for disability retirement status was approved.32 This new information discredited Conseco's basis for the denial of LeAnn's claim, which was premised on Conseco's acceptance of the April 21, 2003 disability date provided in the November 18, 2003 WOP claim form.
